Vacation Request Forms

Students planning to miss school due to a vacation or trip must obtain prior administrative approval.  A form for such absences can be obtained by using the links below or requesting one from the main office.  The completed form must be returned or emailed to Mrs. Beth Whelan: bwhelan@oxfordasd.org five school days prior to the absence. The principal can approve only a total of five days for the school year.
